Physiological jaundice appears >24 h, peaks day 3-5 in a WELL, feeding baby. Red flags for pathological: onset <24 h, weight loss, lethargy/poor feeding, unwell. Well baby -> measure serum bilirubin and plot on age-hour nomogram (reassure if below line); red flags -> serum bilirubin +/- septic screen; above treatment line -> phototherapy.
