A young adult with recent diarrhoea, orogenital ulcers, asymmetric arthritis and dactylitis (sausage finger) fits reactive arthritis (HLA-B27 associated). Behcet causes recurrent ulcers without dactylitis and is not post-infectious; HIV does not cause specific dactylitis. The post-GI trigger and dactylitis distinguish reactive from psoriatic arthritis.
