In an RA patient on methotrexate with localised shoulder pain, targeted ultrasound is the imaging of choice: it is cheap, dynamic and detects synovitis, effusion and soft-tissue pathology (rotator cuff tendinopathy, subacromial bursitis) with MRI-comparable accuracy.
