Teaching point — how the wording changes the answer
A 12-month-old with 2 months of vomiting and head circumference accelerating to the 98th percentile suggests raised intracranial pressure. Urgent CT brain is needed to exclude a structural lesion or hydrocephalus before any invasive procedure; LP is contraindicated with an undiagnosed lesion (risk of coning).
