A closed tibial (long-bone) fracture followed by intense progressive swelling and pain out of proportion is acute compartment syndrome, a surgical emergency requiring immediate fasciotomy. Elevating the leg worsens ischaemia by reducing arterial inflow; antibiotics are not indicated without infection.
