A corkscrew oesophagus on imaging indicates distal/diffuse oesophageal spasm; confirm with manometry and exclude achalasia/reflux. First-line therapy is smooth-muscle relaxants such as calcium channel blockers (diltiazem) or long-acting nitrates.
