When a patient with migraine features collapses after exertion, treat the physiological derangement before the headache. Correct hypotension with IV fluids and hypoglycaemia (BGL ~3.5) with IV glucose; a triptan is wrong until the patient is haemodynamically and metabolically stable. If hypotension dominates, fluids come first.
