Post-hysterectomy -> oestrogen-ONLY HRT (no progestogen needed, no uterus). Local symptoms (atrophy/dyspareunia) -> topical vaginal oestrogen. Asymptomatic menopause (labs only) -> no treatment. Continue systemic HRT for vasomotor symptoms with annual review/mammogram.
