An elderly patient found down with bradycardia (HR ~45) on propranolol and a rivastigmine patch: rivastigmine (a cholinesterase inhibitor) and beta-blockers cause bradycardia. First remove the rivastigmine patch (remove the offending agent); give atropine if bradycardia is significant or persistent.
