Cognitive decline with refusal / collateral from son

When an elderly patient (MMSE ~21-23) minimises her memory loss but the son is worried, do not label dementia from a single score: interview the patient alone to assess cognition, capacity and possible coercion/elder abuse, and reschedule if she is agitated.
