A stable patient with a contained appendicular mass/abscess is managed conservatively first with IV antibiotics (Ochsner-Sherren); larger abscesses (~5 cm) often need percutaneous/radiological drainage. Perforation with peritonitis or rupture is a surgical emergency requiring laparotomy.
