Standard scabies treatment is two permethrin applications 7 days apart; post-treatment itch is common and persists for weeks. If itch alone, reapply at day 7 and/or use topical corticosteroid; crusted/refractory or community outbreaks (e.g. Aboriginal communities) may need oral ivermectin.
