Refractory VF cardiac arrest

In a shockable arrest, after the third shock (persistent VF post two shocks with IV access) give a third defibrillation, then adrenaline 1 mg and amiodarone 300 mg per ALS. The immediate next step after a rhythm check showing VF is to shock again.
