Mild hyperkalaemia (~5.5-5.6) without significant ECG changes is managed conservatively — stop the offending drug (ACE inhibitor) and consider cation exchange (resonium). Significant ECG changes warrant IV calcium for cardioprotection and insulin-dextrose to shift potassium.
