Unexplained iron deficiency anaemia with low vitamin D raises coeliac disease — check anti-tissue transglutaminase. In an asymptomatic patient with a coeliac sibling but negative serology, reassurance (or HLA DQ2/DQ8) is reasonable. In the elderly, unexplained iron deficiency is colorectal cancer until excluded.
