Risk category depends on the affected relative's age. One first-degree relative diagnosed at >=60 (Category 1, near-average risk) = biennial iFOBT from 45-50. A first-degree relative diagnosed <55 raises the category and warrants colonoscopy. A negative FOBT does not substitute for indicated colonoscopy in higher-risk people.
