Bradycardia in a patient on rivastigmine (cholinesterase inhibitor) +/- beta-blocker -> REMOVE the offending drug first (rivastigmine patch); atropine/pacing if unstable. Recurrent exertional syncope + bradycardia -> arrhythmia -> pacemaker (sick sinus / heart block).
