Teaching point

Acute myocardial ischaemia/infarction is the commonest cause of VF in adults (also electrolyte disturbance, long QT/torsades, hypoxia, toxins). VF is a shockable arrest rhythm: immediate defibrillation with CPR and adrenaline; an implantable defibrillator is for secondary prevention, not acute management.
