Resolved chest pain with normal serial ECG + troponin -> low risk -> discharge with safety-net / outpatient stress test. Post-PCI severe pain + inter-arm BP difference -> suspect aortic dissection -> control BP (IV labetalol/GTN) + urgent imaging, NOT thrombolysis.
