HIV-positive patient refusing to inform spouse

First encourage the patient to disclose his HIV status to his wife himself. Public-health-assisted partner notification (or, last resort, breaking confidentiality) only follows persistent refusal; you do not disclose to the wife immediately even though she is also your patient.
