Minimal vaginal bleeding with a normal-sized uterus, positive hCG, closed os and no passed tissue = threatened miscarriage. Fetal viability is established by transvaginal ultrasound (visualising a fetal pole with cardiac activity); quantitative hCG does not show viability.
