Young + sudden pleuritic SOB + reduced breath sounds + hyperresonance -> spontaneous pneumothorax -> CONFIRM with erect CXR if stable. Small primary + minimal symptoms -> high-flow O2 + observe; large/symptomatic -> aspiration or chest drain. Tension (haemodynamic compromise) -> immediate needle decompression (NO imaging first).
