In a testicular mass, gynaecomastia is a paraneoplastic feature suggesting malignancy. Diagnose with ultrasound; with raised tumour markers, stage with CT chest/abdomen and treat by radical (inguinal) orchidectomy. A separate, soft cord swelling (hydrocele) is benign - reassure or aspirate.
