Bilateral middle-ear effusion with cervical lymphadenopathy after a URTI is a reactive viral process (OME); manage with observation/watchful waiting. Antibiotics are not indicated unless acute bacterial otitis media supervenes; ENT referral only if effusion/nodes persist beyond 6-12 weeks.
