Hepatomegaly + raised JVP + murmur + poor feeding + crackles in an infant = CONGESTIVE HEART FAILURE (e.g. large L-to-R shunt / VSD), not primary lung disease. Bronchiolitis = viral prodrome, wheeze/crackles, NO murmur, NO hepatomegaly.
