McMurray: pain/click on external rotation -> medial meniscus; internal rotation -> lateral. Diabetic foot, probe-to-bone positive -> osteomyelitis -> bone biopsy for culture (MRI if uncertain). Pain + pulselessness + paraesthesia (6 Ps) -> acute limb ischaemia -> urgent vascular referral.
